Leeds PCT's Health Trainers are helping people to make positive changes to their health
Twenty-four dedicated Health Trainers are working in local communities to provide free, one-to-one support to people who want to find out how they can improve their health through simple lifestyle changes ?V such as joining a local gym. The Health Trainers are all local people who have experience and an understanding of what it means to live in or be part of the community they work in and are knowledgeable about local community-based services. They are recruited and managed on behalf of the PCT by local community and voluntary organisations who have first hand knowledge of working with local people to improve their overall quality of life.
The role of the Health Trainers is to:
- Boost motivation and improve confidence
- Train people in skills to actively set their own behavioural goals, and manage their own behaviour and the events or circumstances that may influence this
- Help people focus on their achievements and positive aspects of changing
- Provide help in finding and attending other services and support
Health Trainers can provide personalised support over six to eight sessions, tailored to the specific goal of the individual such as reducing stress levels, weight management, taking up more physical activity or reducing smoking. The discussions include looking at what action they can take to improve their own health which fits in with their lifestyle through the development of a personal health plan. It will also cover further help the person may want to seek from local services including referral, where appropriate, to more specialist services.
Who can see a Health Trainer?
Anyone over 16 and living in the following areas can access a Health Trainer for free support in their local area by contacting the relevant organisation: Armley, Bramley, Beeston, Belle Isle, Burmantofts, Chapeltown, Gipton, Harehills, Holbeck, Lincoln Green, Little London, Middleton, Middleton Park, Osmondthorpe, Pudsey, Richmond Hill, Seacroft and Wortley.
People can self refer or be referred by a professional.
